Article 11 of the Law No. 6698 on the Protection of Personal Data ("Law") grants certain rights to personal data subjects. Pursuant to Article 13 (2) of the Law, your applications submitted to us will be responded to within thirty days from the date of receipt of your request, depending on the nature of the request. If the information and documents you submit are incomplete or incomprehensible, we will contact you to clarify your application.

Declaration of Kaizen

This application form has been prepared to determine your personal data, if any, processed by Kaizen and respond to your application within the legal period. In the event that the information regarding your requests submitted within the scope of the form is incomplete, inaccurate, and outdated or an unauthorized application is made, Kaizen does not accept any responsibility for the requests arising from such inaccurate information or unauthorized application. All information required in this form must be provided in full. You are responsible for any unlawful, misleading or false applications. 

If your application is answered in writing, we will not charge a fee for up to ten pages, and a processing fee of 1 Turkish Lira will be charged for each page over ten pages. If the answer to your application is given in a recording medium such as CD, or USB, the fee we will charge will not exceed the cost of the recording medium.
